UniCare offers major medical health insurance to Indiana companies with 2-50 employees. UniCare emphasizes quality care, freedom of choice, and an extensive network of physicians, hospitals, and other health care providers. When using UniCare participating Providers, your employees and their dependents are given negotiated discounted rates which, in turn, provide them with the quality care they deserve while reducing their out-of-pocket costs.  
How UniCare Participating Providers Save You Money
UniCare reduces health costs without sacrificing quality of care or choice of health care providers.  
When you choose a provider from the UniCare network, UniCare pays a higher percentage of the cost because its providers have agreed to accept the lower negotiated rate. Therefore, your out-of-pocket costs will be lower.  
If you choose a doctor or hospital that is NOT part of the network (a nonparticipating provider), UniCare will still provide you with benefits. However, UniCare will pay a smaller portion of the bill, and the bill may be substantially higher than with participating providers. You will be responsible for any balance due and your out-of-pocket costs may be significantly higher when you use a nonparticipating provider.  
Composite Rating Option 
Composite rating is an additional rating option for larger size groups. To choose composite rating, your group must have 25 or more enrolling employees. When you choose composite rating for your group, your employees will have the same rate for each benefit plan in one of four family categories: Employee Only, Employee and Spouse, Employee and Children and Family.
