Welcome Members

As a member, your health plan is administered by UniCare. 
If you want information about your specific plan, consult your benefit manager at work or call customer service at the toll-free number on your identification card. Up-to-date information about your employer's benefit plan is kept on our computer system and is readily accessible to our customer service representatives who can answer many of your questions with one easy phone call.

Customer Service can give you general information about your health plan or explain specific benefits, including:  
any copayments, deductibles or coinsurance;
how to get care away from home;
what to do in an emergency;
how to file a claim or give you the status of a claim.
 
Online Member Services
Members can serve themselves to find personalized plan information and answers to benefits questions 24 hours a day seven days a week. Members can log onto www.UniCare.com and based on their type of coverage:  
Check on the status of a claim or deductible
View their Explanation of Benefits (EOB)
Request a claim form or replacement ID
Reference information related to your plan coverage
Find a network doctor, hospital, dentist or pharmacy in our Provider Finder database
 
Your Identification Card Is Your Passport To Your Health Benefits.
It identifies you as covered by UniCare whenever you need to use your health care benefits, and will enable network providers to take care of all paperwork so you will not have to file a claim.
It gives the name of the network through which you can receive quality care with the least out-of-pocket expense.
It provides useful telephone numbers, including those for customer service and managed care services, who must usually pre-certify services such as inpatient hospital admissions.
It will sometimes indicate the copayment you must pay physicians for each office visit.
It will sometimes indicate if you have other than medical coverage through UniCare, such as dental or prescription drug.
