Health Education Programs to Help Members Stay Healthy 
UniCare Health Plan of West Virginia, Inc. (UniCare) wants to help our members lead healthy lives. We understand the importance of giving you preventive health care guidelines and educational programs that are useful to you. Diabetes Program  
The goal of the Diabetes Management Program is to educate members about managing their diabetes by promoting an interactive approach to diabetes care by using action plans and behavior modification. 
 
Asthma  
The goal of the Asthma Management Program is to educate members on developing or improving their asthma self-management skills by facilitating member and physician interaction.  
 
Prenatal Programs 
The goal of the Prenatal Program is to educate pregnant women on the importance of early prenatal care for healthy birth outcomes.

Cardiovascular  
The goal of the Healthy Habits Count for Your Heart program is to help improve members’ self-management skills and adherence to treatment plans for their cardiovascular condition(s)  
 
Health Education Classes 
Want to learn more about your health? Take one of our FREE classes. Health education classes are available on a self-referral basis. Class topics include prenatal care, diabetes, asthma, and many more. Call the Customer Care Center (CCC) at 1-800-782-0095 or your local Community Resource Center (CRC) at 1-888-611-9958.
