Providers may receive referrals from our two offices in San Diego, CA and Denver, CO. Depending on the situation, the office may use one of the referral models below:  

Referral Models

Model I 
EAP contacts provider with referral
EAP sets up appointment with provider or requests provider to contact client for appointment
EAP sends confirmation packet to provider
 
Model II 
Client is given provider names and phone numbers
Client contacts providers and makes provider selection
Client identifies they are covered by UniCare EAP and sets up appointment with the provider
Client calls and supplies EAP with date of first scheduled appointment
EAP sends confirmation packet to provider
Provider calls EAP to report first session date
 
 
Note: Under both referral models, providers receive a confirmation packet of forms and information from the EAP prior to the first appointment. 
If the confirmation packet is not received prior to first appointment please contact: 
Model I – UniCare EAP 800-728-9492
Model II – Client (to instruct the client to call the EAP and report provider name and first visit date)
Providers should confirm at time appointment is made how session will be managed, whether through EAP or Behavioral Health services.
